What happened there was a chain reaction.
One injector was not working correctly and got really lean burning a hole in that piston which caused excessive crankcase pressure to puke oil out of the valve cover on to the hot headers.

EDIT: The story with photos is still there ... just not one of the lead articles.

Getting held at 7500 forever didn't help either.
